BLEEDING GUMS IN PERIMENOPAUSE: 4 THINGS THAT ACTUALLY HELP

The World Health Organisation has recommended the miswak as a genuine alternative to a toothbrush since 1987. And a microbiome study found that a simple piece of sugar free gum can cut plaque by 20% in two weeks.

Last week, sister, I explained why your gums bleed in perimenopause. If you missed it, the short version is this. Oestrogen, your builder, comes and goes, and your gums become more sensitive and less defended than they used to be.

This week is the part you have been waiting for. What do you actually DO about it, beyond the brush and the floss you already own?

Four things. Each one earns its place from a different lens, mainstream medicine, holistic care, and our own Sunnah, the way I always look at things.

Why these four, and not just "brush harder"?

Bleeding gums in perimenopause are mostly a story about plaque your mouth can no longer clear as efficiently as it once did. So every practice below has one simple job. Reduce the bacteria sitting on your teeth, your tongue, or in your plaque, before it gets the chance to irritate gums that are already working harder than they should have to.

None of these replace your toothbrush. They sit alongside it.

Oil pulling with coconut oil

Swish one tablespoon of virgin coconut oil for ten to fifteen minutes, first thing in the morning, before you eat or drink anything. Spit it into the bin, never the sink, because it sets and blocks your pipes.

A systematic review of randomised trials found oil pulling reduced bacterial colony counts and plaque scores. But I will be honest with you, sister, the studies are small, and dental bodies do not recommend it as a stand-alone treatment. So treat oil pulling as a lovely add-on, never a substitute. Natural does not equal to safe, and it certainly does not mean instead of.

Tongue scraping

Most of the bacteria behind bad breath and plaque are not hiding on your teeth at all. They sit at the back of your tongue.

Use a copper or stainless steel scraper each morning. Pull it gently from the back of your tongue to the front, four or five times, rinsing between strokes. A study on tongue scraping found that doing this twice daily for one week measurably reduced the bacteria living in the mouth. Thirty seconds, and you are done.



The miswak

This is the one I love most, because our deen got here first. The World Health Organisation endorsed the miswak back in 1987, and a scoping review of the clinical evidence found it reduces plaque and gum inflammation to a level close to brushing alone, with the best results coming from using it alongside a toothbrush, not instead of one.



Our beloved Prophet, peace be upon him, said that were it not for fear of overburdening his followers, he would have ordered the siwak before every prayer. SubhanAllah, he knew this fourteen centuries before any laboratory confirmed it. Keep one by the sink, use it before wudu, and let cleaning your mouth before salah become a small act of worship as well as good hygiene.

Xylitol

For the moments you cannot brush. After lunch at your desk, after the children's snacks, in the car.



Reach for a gum or mint sweetened with xylitol, checking it sits near the top of the ingredients list. A microbiome study found xylitol gum reduced plaque by 20%, and lowered the bacteria most linked to gum disease, because the bacteria that normally feast on sugar simply cannot use it the same way. One ordinary habit, working quietly in your favour.

An inspiring story

Remember Sara, from last week's article? Once she understood why her gums had started bleeding, she asked me the obvious next question. "Dr Nikita, what do I actually do, beyond the basics?"



We added two of these four practices to what she was already doing. The miswak, kept by her sink and used before Fajr, and a tongue scraper, thirty seconds each morning. Nothing dramatic, nothing expensive.



Within a few weeks, the bleeding she had been quietly worrying about had settled. Now everyone is different, and I never promise an outcome. But it shows you what becomes possible once you know exactly where to put your effort.



My last thoughts

None of these four things are a miracle cure, and I would never tell you that. They are simple, evidence-aware, faith-aligned habits that work quietly alongside the basics you already know.

This body is an amanah, a trust Allah gave you to look after, so spending thirty seconds on a tongue scraper or reaching for the miswak before wudu is never vanity. It is care, in the same spirit as the rest of our deen.
